12.5.413 looks like it should hit the spot:

Houdini 12.5.413: Fixed a bug where running multiple Houdini sessions would either crash Houdini or produce icon cache corruption warnings.

Houdini 12.5.413: A corrupted icon cache should no longer cause Houdini to crash, though a corrupted icon will not display properly.

Anyone else still having icons issue? I do, I a using build 371.

Well, if you're paying for it then you should likely email Support.

Also, if you've ever used professional software before you'll know that there is no software that is free of bugs. The more complex the software the more chance of a bug.

Have you tried just deleting the mentiond files? I did, houdini recreated it, and now works like a champ.
