Added new consolidate option to PolyExtrude "All Per-Point". This option behaves just like "All" unless "Keep Shared Points Together" is toggled on. In this case it fuses points extruded from each point on the extruded faces. This option is less sensitive to numerical instability than "All" is when "Keep Shared Points Together" is used.
